What we offer
Community Nursing Care
We offer services for NDIS participants who require a higher level of care, which is precisely what we deliver. Our community nursing care services provide advanced levels of nursing care from proficient nurses within the participant’s own home, ensuring comfort and convenience. This includes:
- Medication Management
- Palliative Care
- Simple and Complex Wound Management
- Continence Support and Advice
- PEG Meal Assistance
- Tracheostomy and Ventilator Care

- Care After Hospital
- SPC and Cathether insertion

Daily Living Support
It might be difficult for some individuals to get around and perform tasks independently. Daily living support ensures the participant is safe and comfortable in their home environment. We can assist with
- Bathing
- Dressing
- Grooming
- Toileting
- Eating and drinking
- Other day-to-day tasks
Psychosocial Support
Psychosocial support is a non-clinical, community-based support that focuses on developing personal capacity in practical and vocational skills, friendships and family connections, financial management, sobriety and other related capabilities.
We facilitate social activities, such as social walking groups. Please take note that this is not an emergency response service; however, we will create a plan with measures to keep you safe in the event that you are feeling unwell.
Community Access
SNG Healthcare is aware of the importance of NDIS programs for improving mental health and promoting independence and belonging for people with disabilities. We will support you and connect you with local groups plus continue to take part in activities that you love. We provide transport to medical appointments, social events such as
- Watch a Film or Attend a Concert
- Plan an Outing or Vacation
- Visit a Beach or Park for a day trip
- Join a social group to Meet New People and Socialise
- Attend Community Events in the Local area
Positive Behaviour Support
Positive behaviour support is a structured approach to helping individuals to develop life skills and the capacity to manage their own emotions. By teaching strategies to deal with negative emotions in constructive, rather than damaging, ways, individuals can lead more fulfilling lives with improved relationships at home and in the workplace.
- We aim to comprehend the triggers behind behaviours, which may be psychological, sensory or functional in nature
- We take into account any health issues that could be impacting the participant's behaviours
- We develop and review behaviour support plans to provide a structured approach to address challenging behaviours
- We identify and implement preventive strategies to help minimise the occurrence of challenging behaviours, and also offer assistance when behaviours do occur.
Allied Health
Allied health can make all the difference in helping individuals with disabilities avoid long-term care facilities or prevent physical challenges from becoming debilitating. We work collectively with approved NDIS services to provide the following

Accommodation and Respite
To qualifying NDIS participants, we provide supported independent living supports.
All of our developments adhere to the specifications needed for SDA housing, guaranteeing the
